This symposium brings together leading researchers to examine how evidence-based psychological treatments are adapted, implemented, and sustained in real-world clinical and community settings, with a focus on balancing fidelity, fit with the context, and equity. Across diverse populations, disorders, and service contexts, the talks highlight common challenges and innovative solutions in translating empirically supported treatments into routine care.
Allison G. Harvey, Ph.D., presents findings from a large Hybrid Type 1, cluster-randomized trial comparing Standard versus Adapted versions of the Transdiagnostic Intervention for Sleep and Circadian Dysfunction (TSC) delivered by community mental health providers. Results demonstrate that while both versions were effective, the adapted intervention showed greater uptake and lower dropout, underscoring the value of stakeholder-informed adaptation and facilitation-based implementation strategies.
Shannon Wilsey Stirman, Ph.D., examines how CBT is actually delivered following training and consultation in routine care. Using observational ratings of therapy sessions, this presentation will characterize common CBT strategies, therapist competence, and patterns of treatment adaptation, offering critical insights into fidelity, flexibility, and implications for training and sustainment.
Candice Monson, Ph.D., traces the evolution of Cognitive-Behavioral Conjoint Therapy for PTSD (CBCT for PTSD), from its original protocol to numerous adaptations across populations, formats, and delivery systems. Her presentation highlights how systematic adaptation has expanded reach while maintaining core mechanisms, as well as lessons learned from large-scale dissemination efforts in the U.S. and internationally.
Debra Kaysen, Ph.D., presents results from a parallel effectiveness trial testing culturally adapted PTSD and substance use treatments among rural Native American participants. Findings demonstrate that trauma-focused and substance-focused pathways both reduce HIV risk behaviors, with particularly strong benefits associated with PTSD symptom reduction, emphasizing the importance of culturally grounded, community-delivered interventions.
Together, these presentations illustrate how thoughtful adaptation, measurement of real-world delivery, and partnership with communities can enhance the public health impact of evidence-based psychotherapies. Our discussant, Professor J.D. Smith, will integrate cross-cutting themes and highlight future directions for implementation-effectiveness research.
